Screening, Brief Intervention, and Referral to Treatment (SBIRT)
Screening, Brief Intervention, and Referral to Treatment (SBIRT) is an evidence-informed, system-level approach that helps improve health outcomes for people with HIV by addressing risky substance use. SBIRT integrates three key steps into care:

Screening to assess the severity of alcohol and drug use.

Brief intervention to raise awareness of risks and strengthen motivation to change behaviors.

Referral to treatment for further evaluation and specialized care when needed.
Adapted for HIV care settings with input from experts and community members, SBIRT was tested through HRSA’s Ryan White HIV/AIDS Program (RWHAP) E2i initiative and shown to support healthier outcomes.

Tailored Motivational Interviewing (TMI)
Tailored Motivational Interviewing (TMI) is an evidence-based intervention that uses brief, collaborative counseling sessions to support young people with HIV—including Black gay, bisexual, same-gender loving, and other men who have sex with men (MSM). Instead of directing change, TMI guides clients through conversations that build motivation and confidence to stay engaged in HIV care, take medications as prescribed, and improve overall health behaviors.
Developed by HIV and motivational interviewing experts with community input, TMI was tested through HRSA’s Ryan White HIV/AIDS Program (RWHAP) E2i initiative and shown to improve care engagement and adherence outcomes.

Project CONNECT
Project CONNECT is an evidence-informed intervention designed to help people with HIV—particularly Black gay, bisexual, same-gender loving, and other men who have sex with men (MSM)—successfully engage in HIV care. The program uses early orientation, relationship building, and consistent personal contact to connect clients to treatment.
Newly diagnosed clients, those re-entering care, or those transferring from another clinic meet with a linkage coordinator within five business days. During this orientation, clients complete a biopsychosocial assessment, learn what to expect from care, and schedule their first primary care visit. Linkage coordinators accompany clients to their first appointment, maintain regular check-ins, and provide coaching, case management, and referrals to supportive services as needed.

TXTXT
TXTXT is an evidence-informed intervention that helps young people with HIV—especially young Black gay, bisexual, same-gender loving, and other men who have sex with men (MSM)—stay on track with their HIV treatment. The program uses daily, automated, and personalized text messages to remind and encourage clients to take their HIV medication as prescribed. In addition, participants receive adherence education, counseling, and referrals to supportive services when needed.
Developed and tested through HRSA’s Ryan White HIV/AIDS Program (RWHAP) E2i initiative, TXTXT has been shown to improve medication adherence and health outcomes for youth with HIV.

Healthy Divas
Healthy Divas is an evidence-informed intervention designed to improve health outcomes for transgender women with HIV. Created in partnership with HIV experts and community members, the program focuses on building personal strengths and resilience, supporting gender affirmation goals through peer-facilitated counseling, and reducing barriers to care.
Tested as part of HRSA’s Ryan White HIV/AIDS Program (RWHAP) E2i initiative, Healthy Divas has been shown to empower participants and strengthen engagement in care.
