Collaborative Care Management (CoCM)
Resource
Collaborative Care Management (CoCM) is an integrated behavioral health intervention that improves outcomes for people with HIV experiencing depression and other common psychiatric disorders. In this team-based model, a primary care provider, behavioral health care manager, and psychiatric consultant work together using validated measures and a client registry to track progress and improve care.
The Implementation Guide—developed through Using Evidence-Informed Interventions to Improve Health Outcomes among People Living with HIV (E2i)—provides practical tools, resources, and step-by-step guidance to replicate CoCM in Ryan White HIV/AIDS Program (RWHAP) settings and other HIV service organizations.
